Specification
The Patient Navigator will be required to help patients navigate the clinics in order to access required services at different service points, build working relationships, help solve problems and provide any other support to the patients

TECHNICAL RESPONSIBILITY:

oSupporting patients and clients navigate facilities for different service points.
oBeing the contact person for patients traced and referred from community services.
oEngaging with WBOTs regarding outcomes of patients being traced to ensure that information is captured on Tie.net
oBuilding and maintaining effective working relationships with all the stakeholders at facility level (Aurum and DOH Healthcare team, Patients and Community resources providers)
oAssisting to prepare files (pre-retrieval) for booked patients, ensuring all clinical reports are in the patient files.
oLiaising with the decanting teams at facility level and provide required support.
oImproving coordination of patient care and reduce barriers, find creative solutions to patient barriers.
oBuilding professional relationships with the healthcare team by learning about the role of each team members and by introducing services available in the facilities.
oBeing a reliable source of information for patients

Requirements

Education:

oQualification/Professional Membership:
oGrade 12
oBasic computer skills (an added advantage)
oCertificate in HIV/AIDS and TB Counselling is highly advantageous

oOther Requirements:
oDemonstrable, thorough understanding of HIV/AIDS epidemic
oAbility to read and write activity reports
oGood communication and the ability to establish effective, trusting working relationships with patients
oDemonstrable experience in counselling and Health promotion is highly advantageous
oDemonstrable empathy, compassion and an understanding of the cultural and socioeconomic backgrounds in the community
oAbility and willingness to learn
oAlways maintain positive attitude
oEffective interpersonal and communication skills (written and verbal)
oGood personal time management
oAbility to work independently and in a team
