Four people have died, and at least 255 others have suffered injuries following three separate bus-related incidents in the Free State.
Emergency teams responded to the incidents on Saturday morning, 1 August 2026. The crashes occurred in the Xhariep and Thabo Mofutsanyana districts.
The three emergencies placed enormous pressure on responders. However, Free State Emergency Medical Services worked with partner agencies to assess, treat and transport patients to appropriate health facilities.
Bus overturns near Smithfield
The first incident occurred at approximately 4.30 am near Smithfield in the Xhariep District.
A bus overturned at the R701 T-junction near Voortrekker Street. The crash left two people critically injured and two others with serious injuries.
In addition, emergency teams treated 64 people for minor injuries. Therefore, the Smithfield crash accounted for 68 injured patients.
Fortunately, officials reported no fatalities at the scene.
Emergency personnel completed their work and cleared the area. However, authorities have not yet announced what caused the bus to overturn.
Investigators will need to examine the condition of the bus, road conditions and other available evidence before establishing the cause.
Three buses involved in Bethlehem-Reitz collision
While teams managed the Smithfield scene, another major emergency unfolded on the Bethlehem-Reitz Road.
The multi-vehicle collision involved three buses and resulted in the morning’s largest number of injuries.
Emergency teams classified two patients as critical and five as seriously injured. A further 180 people suffered minor injuries.
The Bethlehem-Reitz crash therefore left at least 187 people requiring medical assessment or treatment.
Despite the high number of patients, officials confirmed that nobody died in this incident.
The involvement of three buses created a complex scene for emergency workers. As a result, responders had to assess many people quickly and identify those who needed urgent hospital care.
Authorities have not released details about how the three buses collided. They have also not disclosed the operators, passenger numbers or destinations of the vehicles.
Four killed at Reitz-Tweeling-Frankfort crossing
The third incident brought the morning’s most devastating news.
A bus and a light motor vehicle collided at the Reitz-Tweeling-Frankfort crossing in the Thabo Mofutsanyana District.
The collision claimed four lives.
Officials did not provide additional information about the victims or confirm whether they travelled in the bus or the smaller vehicle.
Furthermore, the initial emergency report did not include an injury count for this crash.
Authorities will release the victims’ identities after completing the necessary procedures and informing their families.
Free State EMS launches mass-casualty response
Across the three scenes, Free State EMS implemented mass-casualty triage.
This system allows emergency workers to assess many patients within a short period. Responders prioritise people with life-threatening injuries while also ensuring that those with less serious injuries receive care.
EMS personnel worked with law enforcement officers, road authorities and other emergency agencies throughout the morning.
Together, the teams treated patients at the scenes before transporting them to suitable medical facilities.
The three incidents left a combined total of four critical patients and seven people with serious injuries. Meanwhile, 244 patients suffered injuries that officials classified as minor.
Therefore, the first two crashes alone produced a confirmed total of 255 injured patients. Any injuries from the third collision would increase that number.
Health Department sends condolences
The Free State Department of Health expressed its condolences to the families and loved ones of the four people who died.
The department also wished all injured patients a full and speedy recovery.
Meanwhile, relatives may face an anxious wait as hospitals continue treating patients from the Smithfield and Bethlehem-Reitz incidents.
Authorities have not released updates on the conditions of the critically and seriously injured patients.
Investigations into three crashes continue
Investigators must now determine what caused each of the three incidents.
At this stage, authorities have not linked the crashes. They occurred at separate locations and involved different vehicles.
Officials have also not identified speeding, mechanical failure, poor visibility or driver error as causes. Therefore, motorists should avoid sharing unverified claims while investigations continue.
The crashes happened only days after another devastating bus accident near Trompsburg in the Free State. That incident claimed 12 lives after a bus travelling from Cape Town to Johannesburg overturned on the N1.
Consequently, the latest emergencies have renewed concerns about passenger safety and the risks associated with long-distance road travel.
Motorists travelling near crash scenes should reduce their speed and follow instructions from emergency personnel. Drivers should also avoid stopping to photograph incidents because this can create congestion and delay emergency vehicles.
