COMMUNITY SOCIAL WORKER urgently wanted: APPLY HERE

We require the services of a Social Worker. The successful candidate will work collaboratively with St Luke’s Combined Hospice’s Inter-Disciplinary Team (IDT) to provide psychosocial support to client patients, their family members and significant others in the Khayelitsha area.
ENTRY LEVEL QUALIFICATION, KNOWLEDGE AND EXPERIENCE
Bachelor of Social Science in Social Work
A minimum of 3 year’s related experience
Current registration with SACSSP
Unendorsed driver’s licence
Must be computer literate
Fluent Xhosa speaking
Speak a minimum of two of the official languages of the Western Cape
KEY PERFORMANCE AREAS
Provide holistic psychosocial assessment of patients
Provide counselling support to patients and families
Facilitate material support of food parcel distribution and grant applications
Perform crisis interventions when necessary
Compile and submit reports e.g. Psycho-Social Assessments and Client Statistics
Attend meetings and workshops
Perform other duties, as and when so requested, in support of team goals
